Care Management

Elder Care Connection specializes in assisting seniors and their families obtain the health care services that they need in order to live happier and fuller live.  Our clients can reside in private homes, independent or assisted living facilities or nursing homes.

We can help when someone is experiencing:

• Poor or declining physical health
  
• Forgetfulness or confusion
  
• Difficulty bathing, dressing, walking, eating and taking medications independently

Whether a client needs skilled medical care or non-medical personal care, Elder Care Connection can help.  We can evaluate, coordinate and monitor in-home help on a single or long-term basis.  We can provide private home care.  Our staff has been screened for education, prior work history and criminal background.  Care managers can arrange transportation for our clients and assist with errands.  We can also offer assistance regarding Medicaid eligibility.

We appraise the needs of our clients.  We work with families to identify problems, offering options and solutions.  Our services are tailored to meet each client's individual needs, i.e.  assisting families deciding between home care, nursing home and eldercare residences

Families at a distance benefit greatly from our services.  An Elder Care Connection, care manager liaisons with the client's medical providers and family, overseeing care and quickly advising families of problems when they arise.  We provide access to a Registered Nurse 24 hours a day, 7 days a week.
